Our physician-led care teams provide in-home primary care and complementary health care services to patients as quickly and frequently as needed.
Our employed Skilled Nursing Facility (SNF) specialists are onsite and help plan for discharge on day one and work with the house calls care team to help patients experience smooth transitions home and receive follow-up care.
Our care team coordinators and specialists provide around-the-clock support for house calls patients and telemedicine services for rising risk members or patients who prefer virtual treatment.
Our community based, physician- and nurse practitioner-led care teams provide medical, behavioral, social and palliative care to chronically ill patients within their home or skilled nursing facility, while coordinating care across all providers.
