We are passionate about significantly improving the quality of life for our nation's Most Vulnerable Patients (MVPs)—a population that often suffers from multiple chronic conditions, has greater difficulty accessing medical care, is a frequent utilizer of the emergency department and hospital, and requires a higher level of care than a traditional office visit can typically offer.
We are 100% committed to helping MVPs live healthier and more fulfilled lives, while also helping reduce their total cost of care. This not only supports patients, but is also an effective solution for our health plan partners.
Today's health care system is not designed for high-risk, medically complex patients. MVPs require a higher-touch, personalized care model to avoid getting lost in a system where they struggle to keep up with the management of their diseases that require multiple doctors, medications and follow-up.

DaVita Health Solutions, through our community-based medical groups and comprehensive care teams, provides medical, behavioral, social and palliative care for a health plan's most vulnerable members—when and where they need it most.
Our physician-led care teams provide in-home primary care and complementary health care services to patients as quickly and frequently as needed.
Our employed Skilled Nursing Facility (SNF) specialists are onsite and help plan for discharge on day one and work with the house calls care team to help patients experience smooth transitions home and receive follow-up care.
Our care team coordinators and specialists provide around-the-clock support for house calls patients and telemedicine services for rising risk members or patients who prefer virtual treatment.
Our community based, physician- and nurse practitioner-led care teams provide medical, behavioral, social and palliative care to chronically ill patients within their home or skilled nursing facility, while coordinating care across all providers.
We help support primary care providers by increasing communication across care settings and working collaboratively to help develop patient care plans outside the office setting.
Services can take place in home or post-acute settings.
DaVita Health Solutions leverages more than 15 years of experience in managing complex patients under at-risk arrangements with payors and risk-bearing entities.
The results from our initial strategic partnership with a large regional health plan mirror that experience. We served approximately 7,000 members in partnership with more than 600 local PCPs and specialists over a two-year period. In the first year alone, we demonstrated measurable results including4:
Harold, exhausted by hospitalizations, medications and specialist appointments following his heart attack. Hospitalized six times in two years. House Calls changed that.
George, faced several challenges in managing his complex health care needs. Developed additional health concerns that weren't getting solved with hospital visits. House Calls gave him new solutions.
Polly, breast cancer survivor with advanced COPD, referred to House Calls by primary care physician. House Calls kept her stable, comfortable and at home to meet her end of life goals.
"We have been able to find an area where we can provide care to patients who wouldn't be able to get it any other way."
- Dr. Joseph Straton, House Calls Medical Director
"I think House Calls is transforming health care, because it is working with patients who need this type of care specifically."
- Dr. Jill Schwartz-Chevlin, House Calls Medical Director
